Join us for a free community information session exploring the importance of Advance Care Planning for people living with dementia and other neurocognitive conditions. This session will be hosted by Clare Cameron, Clinical Nurse Consultant - Consumer Partnerships from Advance Care Planning Australia.

Advance Care Planning helps people consider and communicate their healthcare preferences for the future, while they are still able to make and express decisions. It can provide peace of mind, reduce uncertainty, and help decision-makers and healthcare professionals understand what matters most to the individual.

This session will cover:

  • What Advance Care Planning is and why it matters

  • The importance of planning early following a diagnosis of dementia or another neurocognitive condition

  • Appointing and discussing a substitute decision-maker

  • Documenting healthcare values, preferences and wishes

  • Tips for having conversations with those you trust

  • Available resources and support

Whether you are living with a diagnosis, caring for someone who is, or simply wanting to learn more about advance care planning, this session will provide practical information and guidance in a supportive environment. We hope you can join us for this important community conversation; everyone is welcome. 

Advance Care Planning Australia is a national initiative funded by the Australian Government, Department of Health, Disability and Ageing and their role is to promote awareness of advance care planning across Australia. This event is a partnership between Kingborough Council and ACP Australia for Dementia Action Week.
