Advance care planning involves planning for your future health care. It enables you to make some decisions now about the health care you would or would not like to receive if you were to become seriously ill and unable to communicate your preferences or make treatment decisions.
Advance care planning helps to ensure your loved ones and health providers know what matters most to you and respect your treatment preferences.
Join us on Wednesday 6th August for this workshop with Morning Tea provided
10am to 12pm
The workshop will be facilitated by local Nurse Practitioner, Kelly Arthurs of ANDCare.

What is Advance Care Planning and why it is so important to discuss?
What are the most important aspects to consider with Advance Care Planning
Opportunity to reflect, have a conversation, and commence your own Advance Care Planning journey
What to do next?
This FREE workshop is for all members of the Ryde and Northern Sydney community.
All attendees are eligible for a follow up personal consultation appointment with the Nurse Practitioner, which will take place on Thur 28th & Fri 29th August in North Ryde. One on One Appointments are available at no cost for those eligible for Medicare.
