Event description

This half-day, in person workshop gives people working in CHSP-funded aged care a simple way to understand Advance Care Planning and support older people to make Advance Care Plans. 

Advance Care Planning is planning ahead while the older person is able, in case they are ever too ill or unable to make their own medical treatment decisions.

The session covers:

  • Advance Care Planning in 3-steps
  • Appointing a medical treatment decision maker
  • Talking to people about future medical preferences and values
  • Advance Care Directives


Morning tea provided.


Who's it for?

People working in CHSP-funded aged care who may have conversations with older people about Advance Care Planning. This includes support and intake workers, coordinators, case managers and allied health professionals.


Cost

No cost for CHSP-funded aged care providers.
