Care Finder Services Community Information Sessions

Do you know or are you a vulnerable senior who needs one-on-one support to navigate the aged care system?  
Come and find out more information about the free care finder service funded by The Western Australia Primary Health Alliance.

The care finder service supports vulnerable seniors to access the services they need to live independently at home for as long as possible. It is designed specifically to help seniors access aged care and allied health services.

Come along to these free community information sessions delivered by Umbrella Multicultural Community Care to find out more about the care finder service, who is eligible, and how it could benefit you or someone you know!

• 13 August 2024 - Harrisdale Pavilion - 6pm till 7pm
