Community Conversations is a FREE 1.5-hour education session around advance care and end-of-life planning led by Palliative Care NSW.
Topics covered:
How to navigate end-of-life matters
Understand what palliative care is and how to access it
Start the conversation with loved ones about your values & preferences
How to start your advance care plan, ensuring your health decisions reflect what matters most to you
Local support services available with a member from your local health district

Who: Everyone of all ages is welcome! Active retirees, if you have ageing parents and you need to start thinking about the supports available for them, if you have a serious illness or are currently caring for someone with a serious illness or if you just want to be better informed to manage this stage of your life.
