Community Conversations is a FREE 1.5-hour education session around advance care and end-of-life planning led by Palliative Care NSW in partnership with Far West Local Health District.
Topics covered:
Navigate end-of-life matters
Initiate a conversation with loved ones on what matters most to you
Explore the local support services available to you and your loved ones
Learn how to start your advance care plan
Understand what palliative care is and how to access it
Who: Everyone of all ages is welcome! Active retirees, if you have ageing parents and you need to start thinking about the supports available, if you have a serious illness or are currently caring for someone with a serious illness or if you want to be better informed to support your community.
Cost: FREE! Complimentary morning/ afternoon tea. Free resource kits for all attendees
