Community Conversations is a FREE 1.5-hour yarn about planning for your future health and care in a way that reflects your choices, wishes and values. This is a safe and respectful yarn, led by Palliative Care NSW and Carers NSW, with a health workers from your local community.

Come along and yarn with us about:

• Planning ahead for your health care

• Sharing what matters most to you with your family, carers and health workers

• Making sure your wishes, values, culture, and choices are respected if you become unwell and cannot speak for yourself.

• Understanding your options and making decisions that are right for you and your family

Join us in a safe and welcoming space to ask questions, share stories, and learn more about planning for the future.

Who: Are an active retiree and want to plan ahead, have parents, grandparents or other loved ones who are getting older, are living with a serious illness, are caring for someone with a serious illness; or simply want to know more about the supports available for you, your family and community.
