Event description

Logan Zero would like to invite you to the Hospital Discharges into Homelessness Forum to be held at Kingston Butter Factory on 14 May 2025.

The Logan region is home to many rough sleepers who are experiencing severe health and mental health conditions. The interface between the housing and health systems can be challenging to navigate, with many rough sleepers reporting increasing presentations to emergency departments and discharges from hospital into homelessness. This forum seeks to elevate awareness of the systemic health and mental health issues faced by rough sleepers in Logan. It aims to build an alliance and consensus on priority areas and develop actionable steps to address these critical issues by:

  • Highlighting the current situation in Logan
  • Presenting solutions of existing models
  • Brainstorming local solutions with key stakeholders
  • Developing a working group and advocacy call to action

Keynote Speaker: Dr. Amanda Stafford

Logan Zero is delighted to announce that our keynote speaker will be Dr. Amanda Stafford, the Clinical Lead of the Royal Perth Hospital Homeless Team. Amanda’s team works to improve the continuity of care for patients who are experiencing homelessness through care coordination, in-reach GP care, and collaborative discharge planning.

More speakers will be announced closer to the event.

Please RSVP to this event and join us to advocate for improved health outcomes for rough sleepers within the region.

If you can't attend, please share this invitation with a representative from your organisation.
