Chorus is a for-purpose organisation on a mission to support communities to thrive. We work alongside seniors and people living with disability or mental illness to help them live the life they choose—connected, empowered, and part of their community.
Our unique, self-organising model is built around 20 small, local teams embedded in communities across metro and regional WA, from Joondalup to Albany. These teams are self-sufficient, place-based, and empowered to make decisions close to the customer—enabling more responsive, relational, and locally driven support.
We believe thriving communities are built through connection. That’s why we focus on activating local networks of care—linking customers with neighbours, community groups, and support services—and working in partnership with local government, health providers and other organisations to build sustainable, community-led solutions.
