Join us for a free information session and expo to learn about the new Support at Home Program and connect with local aged care providers.

Time: 10.30am – 12.30pm
Venue: St John's Anglican Church, Diamond Creek
Presentations will include:
Nillumbik Shire Council's Aged Care Navigation and Advocacy Officer providing an overview of the My Aged Care Support at Home Program, including eligibility, the assessment process, and how to engage with a provider.
Services Australia Financial Information Service (FIS) presenting information on income and assets assessments and Support at Home co-contributions.
Local aged care providers showcasing their services and available to discuss care options and individual support needs.
This is a great opportunity for older people, carers, family members and supporters to gain practical information and ask questions about accessing aged care services in their home.
Light refreshments will be provided.
