Education Webinar | Proactive Planning to support Timely, Safe, and Person-Centred End of Life Care at Home
Description
Join us for an informative webinar exploring practical approaches to supporting quality end-of-life care in the home setting. As more people express a preference to remain at home during the final stages of life, health professionals play a vital role in ensuring care is timely, safe, coordinated, and aligned with the person's wishes.
This webinar will feature presentations from Jane Stephens, Clinical Nurse Consultant with the caring@home project, and Elizabeth Forrester, Project Manager for the Quality Use of Community Palliative Care Medicines Project.
Together, they will share insights into initiatives designed to strengthen community-based end-of-life care, improve access to essential medications and resources, and support patients, families, and carers to remain at home when that is their preference.
Drawing on their extensive experience in clinical practice, service improvement, integrated care, and healthcare transformation, Jane and Liz will discuss practical, evidence-informed strategies that support person-centred care, workforce capability, and better outcomes for people receiving palliative care in the community.
This webinar is ideal for health professionals, service providers, aged care staff, community workers, and anyone interested in improving end-of-life care experiences for Queenslanders..
We believe that palliative care is everybody’s business, and we aim to build compassionate communities around people and their families experiencing serious illness, dying, death, caregiving and grief. This webinar is designed for all members of the community including both health and non-health related workers (i.e., healthcare workers, doctors, nurses, social workers, allied health staff, health administrative staff, spiritual care workers, health managers, health educators, pharmacists and paramedics). We also encourage people from diverse backgrounds to attend. These webinars are sponsored by Queensland Health, and supported by Queensland Ambulance Service.
FREE for PCQ members, PCQ Volunteers and QAS Paramedics.
