Dr Aneesha Singh, Geriatrician and General Medicine Consultant with the Community Palliative Care Service, Metro North Health, who will discuss frailty, dementia and how we can prioritise care for older people living with complex health needs. Dr Singh will also share key insights from the Comfort Care Bundle for the Frail.
Kristy Clapp, Clinical Nurse Consultant with Dementia Australia, who will share practical insights into The Nightingale Program, a nurse-led model of care for people living with dementia and the only specialist dementia-based palliative care program in Australia.
Designed for healthcare and aged care professionals, carers, families and anyone interested in supporting older people, this session will offer practical insights grounded in care, compassion and respect for individual choices..
We believe that palliative care is everybody’s business, and we aim to build compassionate communities around people and their families experiencing serious illness, dying, death, caregiving and grief. This webinar is designed for all members of the community including both health and non-health related workers (i.e., healthcare workers, doctors, nurses, social workers, allied health staff, health administrative staff, spiritual care workers, health managers, health educators, pharmacists and paramedics).
