This is your chance to access specialty information that informs the broader supports available for someone diagnosed with a terminal illness/ who is older and frail and for those that are supporting/ caring for them.

Dr. Jane Cooper is a GP form the North-West region of Tasmania. Jane is the principal at the Don Medical Clinic. Located in Devonport, the clinic is committed to providing quality, team based, collaborative, consistent and person-centred health care.

The Don Medical Clinic offers a Keep Well Clinic Program. The program aims to assist patients with a chronic illness to keep well, active and out of hospital. Providing patients with a holistic approach and plan to selfcare.

This Speaker Series will explore how the Don Medical Clinic and Dr Cooper provide systematic and proactive care to patients with a life limiting illness and how the lessons they have learnt can be applied to your workplace.
