Strategies to Support the Transition into Residential Aged Care

Audience: Care and support staff in aged services; older adults and families

The move into residential aged care is often stressful and difficult for individuals and their families.

Poorly managed transitions are associated with depression, anxiety and loneliness in residents.

Better management of the transition experience benefits individuals, their families and the staff who care for them.

In this 60-minute session, you will learn about:

• The broad experience of ageing
• Mental health in older adults, including risk and protective factors
• The influence of transition experiences on health and wellbeing
• Key elements of transition management
• Examples and strategies to support the move into residential aged care

Although the focus is on aged care, the concepts and key messages will be relevant to our personal and professional roles, as we recognise and respond to transitions at home and at work.

About the presenter:

Dr Meg Polacsek is AAG’s Communications, Advocacy and Grants Manager. Meg’s career spans experience in direct care, policy, communications, research and project management roles across aged care, public health and academic settings. As a social gerontologist, she draws on a range of disciplines and methodologies to understand the social, health and personal aspects of ageing, and life course approaches to wellbeing. Her particular interest is in mental health in older adults. She completed a Master of Health Science (Aged Services Management) in 2014 and obtained her PhD in 2018.
