Streamlined, service specific assessments

This interactive webinar will support staff to build on their assessment skills and consider ways to streamline their approach, without compromising quality. We will discuss a range of practical ways to:

• Understand the information available in the client record (via the My Aged Care service and support portal) to inform your assessment

• Build on the information that has already been collected to guide your assessment and planning

• Reduce duplication and improve efficiency for clients, carers and staff

• Focus on what’s important to the client, their priorities and how you can best support them

• Embed a strengths based approach with your assessment conversations

• Introduce the concept of goal setting and care planning to clients and carers

Target Audience

Staff who are actively involved in completing assessments with CHSP clients and carers.

The webinar is tailored to staff working within CHSP funded programs. The content will therefore be linked to the Aged Care Quality Standards (and the impact of the introduction of the Strengthened standards) and will reflect the breadth of practice across different service types and settings

Up to 3 people from the same organisation can attend.

This workshop is for Commonwealth Home Support Programme (CHSP) funded aged care service providers only.
