Streamlined, Strengths Based Assessment & Care Planning

Time: 1pm to 4pm SA, NT| 1.30pm to 4.30pm VIC, NSW, ACT, QLD, TAS| 11.30am to 2.30pm WA
Audience: This workshop is appropriate for both new and established staff who are actively involved in assessment and planning within aged care programs. Program leaders and managers are also encouraged to attend.
This interactive workshop will support staff to build on their assessment and planning skills and consider ways to streamline their approach, without compromising quality.
We will discuss a range of practical ways to:
- Build on the information that has already been collected to guide your assessment and planning
- Reduce duplication and improve efficiency for clients, carers and staff
- Focus on what’s important to the client, their priorities and how you can best support them
- Introduce the concept of goal setting and care planning to clients and carers
- Engage clients and carers to identify relevant and meaningful goals
- Create care plans that are meaningful and useful for you and your clients
The content will align with the requirements of the Strengthened Aged Care Quality Standards and how effective assessment and planning is an essential component of a wellness approach. It will reflect the changes to service specific assessment and planning that relate to the broader aged care reforms (including the introduction of the Single Assessment System, Integrated Assessment Tool and Support at Home program).
This session is also aligned to the foundations of the new regulatory model person-centred focuses such as the unique needs, goals, values, preferences, and dignity of older Australians.

ACIA reserve the right to:
- Remove unauthorised participants from a session if no response is received by the unauthorised attendee within 10 minutes (by Zoom chat).
- Request all participants to turn on their camera

Notification of cancellation or transfer of registration must be by email to enquiry@acia.asn.au. No refund is available for any cancellations received five working days or less prior to the start date, however a substitute delegate is welcome. Please notify ACIA of any substitution as soon as possible. No refund or transfer will be given for non-attendance.
