Support at Home AT-HM Scheme

The Support at Home AT-HM Scheme represents a significant change from the way equipment and home modifications were previously funded under the Home Care Packages Program. This makes it particularly important for Care Partners to understand how the scheme works, what can be funded, and how to apply the current requirements in practice.

This training has already been delivered directly to the majority of Support at Home providers across South Australia. We are now offering this session as an opportunity for organisations to ensure new Care Partners receive training on this important area.

The session may also be valuable for experienced Care Partners. There have been a number of important updates to the AT-HM Scheme since it started, so anyone who hasn’t received training or an update since the data collection period finished in February is encouraged to attend.

This FREE 45-minute session covers:

·      Interpreting the updated Assistive Technology & Home Modifications list (AT-HM List)

·      Funding tiers, funding periods and lifetime caps

·      Pre-approval for funding for items over $15,000

·      Funding AT-HM wrap-around services vs. quarterly budget

·      Sourcing AT-HM funding for clients who need it

About the Presenter:

Michael Peachey is the Director of SA Homecare Therapies, a team of Occupational Therapists and Physiotherapists providing equipment and home modification prescription services to Support at Home recipients. Having delivered AT-HM training to the majority of Support at Home providers across South Australia, Michael brings a practical perspective on the questions, challenges and common areas of confusion Care Partners are encountering as they apply the new arrangements.
