Time: 8:30am – 11.30am

Venue: Mary MacKillop Place, 7–11 Mount Street, North Sydney

Cost: FREE for eligible hospitals and hospital personnel only

Registration: Essential – Limited places available

NDISDA® is pleased to invite hospital social workers, CEOs, hospital executives, discharge planning teams, NDIS liaison officers, allied health professionals, rehabilitation teams and hospital management representatives to our upcoming Sydney Hospital-to-Home Roundtable Forum.

Presented by NDISDA® in partnership with National Hospital-to-Home Care Pathways, this professional education and engagement forum focuses on the challenges hospitals experience when transitioning individuals requiring NDIS-funded supports, disability services, supported accommodation and complex community-based care.

Following our Hospital-to-Home engagement activities across Australia, we are pleased to extend complimentary attendance to Sydney hospitals and hospital personnel, supporting continued professional education, knowledge sharing and practical discussions around improving hospital-to-community transitions.

Why are we hosting this Forum?

NDISDA® is a national education, industry engagement and collaboration body - not an NDIS service provider.

Through our national Hospital-to-Home Engagement Program, we facilitate structured discussions with hospital representatives, government agencies and relevant sector specialists to identify systemic barriers affecting hospital discharge.

We recognise that delays involving NDIS funding, SIL and SDA accommodation, allied health assessments, equipment, support coordination and access to appropriate community-based services can contribute to extended hospital stays and increased pressure on hospital bed capacity.

Our objective is to identify gaps, improve professional understanding and explore practical solutions that support safer, more timely and coordinated hospital-to-community transitions.

NDIS & Disability Hospital-to-Home Care Pathways

The forum opens with a Facilitated Hospital Roundtable & Panel Discussion

This is our primary Hospital-to-Home Roundtable, focusing on the challenges hospitals face when transitioning individuals requiring disability supports and complex community-based care.

Key Roundtable Discussions

  • Reducing Discharge Delays & Hospital Bed Pressures – Identifying barriers to timely and appropriate hospital-to-community transitions.

  • NDIS, SIL & SDA Pathways – Understanding funding arrangements, approvals, accommodation availability and service coordination challenges.

  • Complex Care & Supported Accommodation – Exploring challenges for individuals requiring high-intensity supports, specialised accommodation or multidisciplinary care.

  • Roles & Responsibilities – Clarifying the responsibilities of hospitals, NDIS stakeholders, support coordinators, SIL providers and community-based services.

  • Allied Health, Equipment & Discharge Readiness – Addressing assessments, functional documentation, assistive technology, equipment delays and continuity of care.

  • Communication, Consent & Information Sharing – Identifying gaps affecting safe discharge planning and coordinated transitions.

  • Practical Solutions & Sector Improvements – Discussing opportunities to strengthen professional knowledge, reduce avoidable delays and improve transition outcomes.

  • The forum will be professionally facilitated, with opportunities for hospital representatives to share their experiences and contribute to constructive discussions.

  • Our focus is on identifying systemic challenges and practical improvements — not directing hospitals on how to undertake discharge planning.

Further information

Beyond the NDIS: Understanding icare NSW, Accident, Catastrophic Injury & Lifetime Care Pathways

As part of our broader national commitment to improving professional understanding of government-funded care pathways, we are exploring an additional educational information session involving icare NSW.

This proposed session would provide an introduction to accident and catastrophic injury care arrangements, lifetime care pathways and relevant government-funded support systems outside the NDIS.

Potential areas of discussion include:

  • Understanding relevant icare NSW schemes and their purpose.

  • How accident and catastrophic injury care arrangements differ from NDIS-funded supports.

  • Funding responsibilities, eligibility considerations and care coordination.

  • Hospital-to-community transitions for individuals with catastrophic injuries.

  • Awareness of icare NSW's existing professional education, training sessions and information resources.

Important information

This is a professional education and sector improvement forum, not a provider marketing or referral solicitation event.

Attendance by NDIS and disability service providers is carefully managed to maintain meaningful hospital representation and constructive discussions.

All attending providers are expressly prohibited from approaching hospital personnel to solicit referrals, promote their services or pursue unsolicited commercial opportunities.

Hospital protocols, professional boundaries, confidentiality and established referral processes must be respected at all times.

Our priority is to provide an independent, respectful environment where hospital representatives can openly discuss challenges and contribute to practical improvements.

Who should attend?

This complimentary invitation is specifically for hospitals and hospital personnel, including:

  • Hospital CEOs, Executives and Operations Managers

  • Hospital Social Workers and Social Work Managers

  • Discharge Planning and Complex Discharge Teams

  • NDIS Liaison Officers and Disability Liaison Officers

  • Patient Flow, Bed Management and Patient Access Managers

  • Allied Health Managers and Rehabilitation Teams

  • Mental Health Discharge and Complex Care Coordinators

  • Clinical Nurse Consultants and Hospital Liaison Managers

  • Hospital personnel involved in complex care, discharge improvement and community transitions

Hospital Panel Opportunities – 3 available opportunities to participate in the NDIS and Disability Forum

We are inviting 3 hospital representatives to contribute to our professionally facilitated panel discussion.

This is an opportunity to share hospital perspectives on discharge challenges, patient flow, service coordination and areas requiring improvement.

No formal presentation is required.

Hospital representatives interested in joining the panel are invited to email ndisda@jazcorpaustralia.com.au, including their name, position and hospital.
