The Important Four - Planning for the future!

This education session is for the general community and for those who have attended a 'what matters most' advance care planning session previously and is more suitable for people who have some general knowledge of advance care planning. 

The session will focus on the important documents you need to get prepared and the process behind completing them. 

PCT will joined by special guests The Public Trustee and The Office of the Public Guardian for a great in-depth panel discussion where you will have the opportunity to ask questions and seek further information or resources to consider planning for your future medical care and end-of-life planning

3. Enduring Guardian 

4. Enduring Power of Attorney
