Voices from the field

Health and community services are an essential part of creating age-friendly communities. The Indigo 4Ms program of work is a collaborative approach to addressing the challenges of providing comprehensive, integrated care for older people living in rural northeast Victoria. 

Through co design, the Indigo 4Ms tools were developed as practical devices to equip older people and primary health care workers develop a single comprehensive care plan that maintains or improves functional ability.  

This event will showcase and celebrate the implementation of the Indigo 4Ms tools into the following services:

Alpine Health, Beechworth Health Service, Gateway Health, Tallangatta Health Service and Corryong Health.
