2024 Senior Musical Cast, Crew and Orchestra - Permission and Approval Form

Parents of Cast, Crew and Orchestra members who are in Years 11 or 12, please complete this form to provide consent for your child to visit food stores near the theatre during designated breaks. We ask that you acknowledge that your child will be unsupervised during these times.

Parents of Crew members, by completing this form, you provide consent for your child to participate in the production, acknowledging that attendance is compulsory at all rehearsals and performances as scheduled.
