Managing Heart Failure: From Hospital to Home Course Program 2025

The course aims to help health professionals who care for people with cardiac or chronic conditions to understand the pathophysiology of heart failure, diagnosis and imaging, pharmacotherapy, and use of cardiac implantable devices. 

Participants will gain practical skills in how to optimise medications, manage symptoms and educate patients effectively, running an exercise program and addressing psychosocial issues.

Presenters:

Sessions are delivered by expert heart failure cardiologists, nurses, pharmacists, and allied health clinicians.

Participants: Suitable all healthcare professionals who care for the elderly or those with chronic disease with an interest in cardiology

NB: Course will be delivered face to face and via online webinar. Participants may attend either day of the course or attend the full 2 days.
