Come and join PCT in Hobart for a FREE Advance Care Directive Information Drop-in Session, where our facilitators will be there to provide you with general information on how to fill out an Advance Care Directive and answer any questions you may have relating to your Advance Care Directive and Advance Care Planning.

Complimentary Morning Tea will be provided! These sessions are led by those who attend and encourage open conversations and discussions about planning your future health care. They are suitable for everyone and anyone in the community looking for information or help.
