Advance Care Planning Workshop

If you couldn't speak for yourself, how would others know what choices to make? Advance Care Planning is a way to share your choices with your chosen decision makers and health care workers.

This in-person workshop will cover:

• What advance care planning is

• Thinking about your values and preferences

• Ways to start the conversation with your loved ones

• How to choose a decision maker and document your plan

You'll leave with everything you need to make your plan and share it with others.

This session is for people living in the ACT. It will be helpful for anyone living with a long-term health condition, carers and people with disability.
