Join Palliative Care Victoria at the Craig Family Centre for a supportive and engaging session on Advance Care Planning – the process of thinking ahead about your future health care and making your preferences known.
This 1-hour session is ideal for anyone wanting to better understand how to take control of their health decisions and prepare for unexpected health events. Whether for yourself or to support a loved one, this workshop will empower you to make informed, values-based choices.

How to begin conversations with your loved ones
Choosing the right person to speak on your behalf
What kind of care you would want (or not want)
Legal considerations and how to document your wishes

45-minute interactive presentation
15-minute open Q&A with a trained facilitator from Palliative Care Victoria
