Join Palliative Care Victoria at the Craig Family Centre for a supportive and engaging session on Advance Care Planning – the process of thinking ahead about your future health care and making your preferences known.

This 1-hour session is ideal for anyone wanting to better understand how to take control of their health decisions and prepare for unexpected health events. Whether for yourself or to support a loved one, this workshop will empower you to make informed, values-based choices.

• What is advance care planning and why it matters

• How to begin conversations with your loved ones

• Choosing the right person to speak on your behalf

• What kind of care you would want (or not want)

• Legal considerations and how to document your wishes

• 45-minute interactive presentation

• 15-minute open Q&A with a trained facilitator from Palliative Care Victoria
