Aged Care Connected Conversations - 17 March 2026

Join us for an interactive evening designed to strengthen collaboration across the aged care sector.
Connect with peers, participate in guided discussions, and explore practical ways to strengthen teamwork between GPs, pharmacists, and residential aged care facilities.

CESPHN will share updates on key programs; including General Practice Aged Care Incentive (GPACI), My Health Record, GP Connect, and Aged Care On-Site Pharmacist (ACOP) to support more coordinated and integrated care across aged care settings.

We look forward to hosting meaningful conversations that strengthen partnerships and support better care for residents.

Registrations close: 13 March 2026 (note for late registrations, special dietary requirements may be be able to be catered for).
Target Audience: This event is free and open to GPACI registered GPs, Aged Care Pharmacists/ACOP, RACH Facility and Clinical Managers in the CESPHN region.
