ALICE Breakfast Workshop 1: Understanding the Link Worker Role

ALICE (All-Inclusive Care for Older People) is a new model of care being piloted on the Central Coast. It is as an initiative of Central Coast Local Health District, Hunter New England and the Central Coast Primary Health Network, Central Coast Council and the Department of Regional New South Wales.
ALICE seeks to develop a community-embedded approach to the care of older people living with complex health and social care needs in a neighbourhood setting.
As a part of this, ALICE Link Workers will support older people and carers with holistic (social) care assessments, self-care support, and co-ordinated access to care across primary, community, aged and social care providers.
These ALICE Breakfast Workshops have been established to bring together health and aged care professionals and providers to share their experiences of care coordination, so to advise and support the development of ALICE Link Workers, and to ensure that they add value to existing approaches.
