Privacy Disclaimer

We will only use the personal information you provide to us for the purposes of registering for Antioch 2022 or carrying out our lawful functions. Personal information in this form will not be passed on to third parties and will be securely disposed of one month after this event.

Acknowledgement of Risk

I understand that there are risks associated with involvement in overnight camps and that these risks can not be reduced to zero. I also understand that the management of risk is a shared responsibility between Catholic Youth Team Staff and Volunteers and the camp participants. I know that I am able to ask any questions I like of the Event Coordinator about the activities my child will be involved in, to gain a better understanding of the risks involved. I also recognise that participation in such activities is voluntary on my child’s part and is not a required or mandatory activity. If I feel my child is at risk I may withdraw them after informing and in consultation with the Event Coordinator. I understand that the Catholic Youth Team will identify any hazards that are likely to arise, identify any foreseeable risks at Antioch camp, and will implement correct management procedures to deal with these hazards. I agree that it is incumbent on my child to follow these procedures.

Liability Release

While all reasonable care of managing activities will be taken, in the event of injury, or loss or damage of property arising from unforeseeable circumstances or where the participant has failed to abide by guidelines or rules, the organisers cannot be held responsible for such injury, loss or damage.

Consent Agreement

- I agree to my child taking part in Antioch Camp and have read the information in this form.
- I understand that I am able to ask any questions about the activities my child will be involved in, to gain a better understanding of the risks involved.
- I agree that, in an emergency, any necessary decisions can be made by those in charge if the emergency contacts given in this form cannot be contacted.
- I understand that Antioch camp is smoke-free, alcohol-free, and drug-free.
- I understand that my parish or school may be contacted to discuss the funding of camp fees, and give my permission for this to happen.
