Join a Community Conversation led by Palliative Care NSW in partnership with Highlands End-Of-Life Project and Hunter New England Local Health District.
Community Conversations is an informal education session that brings together palliative care experts and local community members to have open, thoughtful discussions around advance care planning, ageing, dying, and grief. You’ll gain the knowledge and tools in a safe environment to make informed decisions and access the care you need to:
- Navigate end-of-life matters
- Understand what palliative care is and how to access
- Explore the local support services available to you and your loved ones
- Learn how to start your advance care plan
Whether you’re caring for aging parents or a loved one with a life-limiting illness, or simply want to be better prepared, a Community Conversation offers invaluable guidance.
You’ll walk away with a free resource kit and the confidence to navigate life’s most difficult transitions.
