What does care look like in the last 12 months of life?

We want to hear from you.

Join South Eastern Sydney Local Health District, CESPHN and Bayside Council for an open community conversation about what matters most in the last 12 months of life.

Whether you have lived experience as a family member, carer, friend, volunteer, health professional, or community member, your insights are valuable.

Together we’ll explore:

• What are the needs of people, families, and carers in the last 12 months of life?

• What could we do differently to better support people in our community?

• What education, information, or resources would help our community feel more prepared and supported?

• Your ideas will help inform future community education, support, partnerships, and service planning across South Eastern Sydney.
