North Metro-based services providing home and living supports are invited to a design session to explore information transfer solutions to support hospital admission and discharge for people with disability
Hosted by Joondalup Health Campus and the Ready to Go Home project, the design session aims to:
- Identify information critical to support smooth transitions for people with disability
- Explore solutions to enable timely consent to share information processes
- Design an effective mechanism for the transfer of patient information.
