Parental Consent, Waiver & Release of Liability
For Female Martial Arts Training Session (Ages 10–15, Grey Belt & Up)

By registering my child for this event, I, as the parent or legal guardian of the participant, acknowledge and agree to the following:

Assumption of Risk
I understand that martial arts is a contact sport involving physical activity and carries an inherent risk of injury, including but not limited to bruises, sprains, strains, fractures, and other potential injuries. I voluntarily allow my child to participate with full knowledge of these risks.
Release of Liability
I hereby waive, release, and discharge all coaches, black belt instructors, event organizers, volunteers, and associated parties from any and all liability, claims, or demands that may arise from injuries, accidents, or other incidents involving my child during or related to this training session, whether caused by negligence or otherwise.
Medical Treatment Authorization
In case of injury or medical emergency, I authorize event staff to provide or seek appropriate medical care for my child. I understand that I am responsible for any medical expenses incurred as a result.
Acknowledgment of Responsibility
I confirm that my child is physically fit and capable of participating in a martial arts training session. I understand that it is my child’s responsibility to follow instructions, train safely, and communicate any injuries or discomfort to coaches immediately.
I hereby grant permission to Dolci Camps, or anyone authorized by Dolci Camps, the irrevocable right to use any photograph/video taken participating in the activities. Such photographs/video may include the name of the student, title of the project, prize information, name of the school, etc., and may be used on the Dolci Camps and related websites, including social media websites, and in related marketing materials, without media, time, use or geographic limitation.
