These free 15 minutes health checks will cover key health issues (screening for risk of cardiovascular disease, diabetes and mental health), so you can take preventative action to prevent future issues arising. Each appointment will include an assessment and discussion with a registered nurse with experience in preventative and community health. You will walk out with recommendations on key health aspects to be aware of and ideas you may want to consider.
The appointment length is 30 minutes - when you arrive you will be greeted by one of our lovely volunteers who will provide you with a short questionnaire to complete and answer any questions you have, before you head up to level 1 for the check-up.

Consent: by booking in your appointment you confirm that:
- I have read the below 'Client Consent to participate in Community Health Screening Program' and understand the community health screening will not provide direct advice on medical / health issues but general information on health and lifestyle to prevent progression or development of disease. Further, it does not take the place of medical advice as this is being run by Registered Nurses.
- I confirm that I am 18 years of age or older.
Client Consent to participate in Community Health Screening Program
Purpose:
Our Community Health Screening program, conducted by Registered Nurses, is being offered in conjunction with Little BIG Foundation. Our aim is to facilitate the early detection of cardiovascular risk factors to allow timely intervention to minimise the incidence of heart attack and stroke. We also include mental health screening that will enable early intervention for depression, anxiety and stress.
Screening is different from diagnostic testing:
Screening checks everyone in a particular group, so those with early disease can get further tests and treatment. It is different from diagnostic testing, which doctors order if individuals already have symptoms of a disease.
If you already have symptoms or signs of a disease or are at a higher risk of getting a disease, please see your own doctor. Do not wait until you are invited to be involved in a community-based screening program.
Why screening is important:
Screening is important because it:
- reduces the burden of disease in the community, and for individuals it reduces the likelihood of illness and premature death
- detects the disease earlier, on average, than if there was no screening
- improves health outcomes for individuals and their families.
Screening tests are not 100% accurate and the body changes over time. This means it is important to be screened at regular intervals and follow up with your doctor if you are concerned.
This community health screening program is not designed to be an opportunity to be treated for any pre-existing illness or to present if you are feeling unwell in any way. We ask you to please reach out to your local General Practitioner or if it is an emergency to call ‘000’.

We understand that NSW Health Guidelines enable close contacts to no longer isolate, as long as they have no symptoms, perform a daily Rapid Antigen test and comply with current NSW Health guidelines.
However, as we are providing health screening, we ask that if you have a fever or symptoms of respiratory infection (cough, sore throat, shortness of breath) or have been in close contact with a confirmed case of COVID-19 in the last 7 days, that you do not come in for your appointment.
