You are invited to attend our inaugural Healthy Ageing Conference for primary healthcare providers - general practitioners, practice nurses and allied health professionals providing clinical care to older adults in the community. –
This full day of education aims to empower health professionals to support patients to live well for longer in the community.
Featuring presentations and case studies*:
• Prevention of cognitive decline, post diagnostic and ongoing management of dementia
• Addressing what is important to older adults, social prescribing and intergenerational programs
• Chronic disease management including diabetes care, nutrition and falls prevention
