Event description

Join our SOCIAL PRESCRIBING Project to make a positive impact and help improve well-being in your community. Become involved in a pilot project focused on addressing the social needs of older adults in our communities.

Our social prescribing project works alongside local GP’s and other healthcare professionals to identify and connect older adults with community social programs. This approach is ‘human centered’ and will help reduce loneliness, social isolation and foster social connection within the community.

If you enjoy talking to people, discovering what they love to do and have great creative skills to connect people with thriving communities, then this is for you!
