Release information:

I understand that yoga is a physical activity and moves every part of the body. I have cleared any health conditions with a physician prior to participating or affirm that I am able to safely participate.
I also understand that participation in yoga with cats and kittens, where cats roam freely in the class space, can be unpredictable. I understand and accept the risk of injury from interacting with the cats, including, but not limited to, scratches, bites and allergic reaction. I do not hold Enjoy Life Physical Therapy liable for any injury sustained as a result of interacting with the cats. I further understand, any injury related to the cats will be reported to Best Friends and that cat bites need immediate medical attention due to high risk of infection.
As is the case with any physical activity, the risk of injury, even serious or disabling, or even death, is always present and cannot be entirely eliminated. By accepting this waiver of liability, I acknowledge that participating in yoga with kittens and cats class exposes me to possible risk of personal injury. I am fully aware of this risk and hereby release Enjoy Life Physical Therapy and the yoga instructor from any and all liability, negligence or other claims arising from or in any way connected with my participation in yoga class.
If I am pregnant or become pregnant or am post-natal, I verify that I am participating in yoga class with my doctor’s full approval. I realize that I am participating at my own risk.
Registering for this event acts as acknowledgement of this agreement. I assume for myself and any children under 18 years of age (‘minor’) who accompany me, all risks of property damage, illness, personal injury and death incident to my/our exposure to, contact with and/or handling of any animal while visiting the shelter. On behalf of myself and all minors who accompany me, I hereby release the Enjoy Life Physical Therapy from any and all responsibility and liability for property damage, illness, personal injury and death arising out of my/our presence in the shelter and exposure to, contact with and/ or handling of any animal while participating. I further agree to indemnify and hold Enjoy Life Physical Therapy harmless from any and all claims, damages, actions, causes or actions and judgments which may in any way result from my/ our presence in the class and exposure to, contact with and/or handling of any animal while participating.
