Overview

Limbic Risk: Rethinking Incidents, Investigations and Risk Controls

Live Virtual Program | 3 Hours

Most serious incidents do not begin with a rule violation.

They begin with pressure.

Pressure to meet targets.

Pressure to manage competing priorities.

Pressure to make decisions with incomplete information.

When attention becomes overloaded, competent people make mistakes. Hazards are overlooked. Critical decisions degrade. Procedures that normally work are missed.

The challenge is not simply human error.

The challenge is understanding the conditions that make human error more likely.

Limbic Risk provides leaders with a practical framework for identifying those conditions before they contribute to a serious event.

This highly interactive virtual program combines human performance, systems thinking, real incident analysis, leadership discussions and live simulations to help participants recognise the hidden conditions that influence attention, judgement and decision making in high consequence environments.

Practical Outcomes

By the end of this program, participants will be able to:

  • Identify the workplace conditions that increase the likelihood of error, poor decisions and operational drift

  • Recognise early indicators that attention, decision quality and situational awareness are degrading

  • Analyse incidents beyond human error and uncover contributing system conditions

  • Understand how workload, interruptions, fatigue and competing priorities influence human performance

  • Apply the Limbic Risk model to real workplace scenarios

  • Design practical controls that reduce reliance on memory, vigilance and perfect human performance

Program Experience

Why Good People Make Poor Decisions

Explore how pressure, workload and competing priorities influence attention, judgement and decision making.

The Supervisor's Day Simulation

Participants work in breakout teams to manage an evolving operational scenario involving production pressure, staffing challenges, competing priorities and emerging safety concerns.

The exercise reveals how risk develops long before an incident occurs.

Seeing What Others Miss

Analyse real incidents and uncover the operational, organisational and human performance conditions that existed long before the event occurred.

Reframing Investigations

Move beyond human error and uncover the system factors that contributed to failure.

Designing Controls That Work Under Pressure

Learn practical methods for creating controls that support human performance and improve decision making in real world operations.

Interactive Learning Experience

This is not a webinar.

Participants will engage in:

  • Live leadership simulations

  • Breakout room discussions

  • Incident analysis activities

  • Interactive polls and surveys

  • Practical workplace application exercises

Every Serious Incident Has a History

Long before the event, there were competing priorities, interruptions, workload pressures, assumptions and conditions that made failure more likely.

If investigations stop at human error, those conditions remain unchanged.

Limbic Risk helps leaders identify the factors that shape human performance, improve investigations and strengthen risk controls before the next incident occurs.

Places are limited to maintain an interactive workshop experience.

FAQs

  • What is Limbic Risk- for anyone who hasn't come across that term before?

Limbic Risk is the risk created by the human brain when working under pressure. It is a human default, not a personality trait. We need to learn how to design systems to support the brains natural response to pressure and perceived threat.
