Make a start on your Advance Care Directive (ACD)
Event description
An Advance Care Directive is designed to be used if you do not have capacity to make decisions for yourself. This includes treatments you would accept or refuse if you had a life-threatening illness or injury. An Advance Care Directive will only be used if you do not have capacity to make decisions for yourself or to communicate your preferences.
During this small group session we will discuss all the items that go into an Advanced Care Directive and you will be supported to build a robust document to suit your individual needs. We will send you some thought provoking questions prior to the workshop so that you will have time to consider and discuss these things with your family.
