Join us as we showcase best practice advance care planning in aged care: supporting older persons’ right to person-centred care.

    • Hear the consumer and carer voice
    • Network and hear from colleagues who are applying best practice
    • Hear about challenges faced in the real world
    • Learn about available resources, support and education from experts in the field
    • Explore supported decision making, beyond a binary view of capacity

The event will connect health and aged care service leaders, policy makers, peak bodies, consumer groups, academics, and professionals from around Australia to share advance care planning best practice and support the aged and health care sectors moving forward.

The introduction of a new Aged Care Act and new Aged Care Quality Standards in 2025 reinforce the need for a central focus on the older person’s right to autonomy, and to a central and active role in decisions about their care.

Advance care planning is central to person-centred care, providing a means by which a person’s wishes and preferences are considered in care decisions when they are not able to speak for themselves.

The program will feature expert speakers and interactive panel sessions exploring contemporary thinking and best practice in advance care planning for the aged care sector.

    Symposium sessions:
    • Person centred: Hear our voices | Consumer and carer panel with Judith Leeson AM, Director Vector Consultants and Gwenda Darling, Aged Care Council of Elders.
    • Supported decision making: Beyond a binary view of capacity | Keynote presentation by Dr Craig Sinclair, University of New South Wales, followed by a panel discussion where Dr Sinclair will be joined by a number of panellists including Ms Kate Swaffer, Dementia Advocate, and Ms Carolanne Barkla, Chief Executive, Aged Rights Advocacy Service.
    • Advance care planning in the real world | Facilitated panel discussion.
    • How can aged care services and aged care staff be supported to improve their advance care planning practice? | Panel and action planning.
