HOSPITAL PERSONNEL – PRIVATE VIP REGISTRATION

The NDISDA® & Health2Ageducate® Hospital to Home Queensland Forum brings together Queensland hospital personnel, Support Coordinators, NDIS and disability providers, allied health, aged care and community-based services for a facilitated cross-sector forum examining the challenges affecting hospital-to-community transitions.

Hospital-to-community transitions can involve multiple organisations, professionals, funding systems and service pathways. Open communication and a shared understanding between hospitals and the sectors supporting people following hospitalisation are therefore essential.

Contribute your perspective. Hear from across the pathway. Strengthen shared understanding. Be part of the solutions.

Why we are hosting this Forum

NDISDA® has previously facilitated Hospital to Home engagement in Western Australia and Victoria, bringing together hospital personnel with disability, NDIS, allied health and community-based organisations to examine the challenges experienced across hospital-to-community transition pathways.

These discussions have reinforced the importance of open communication and shared understanding between hospitals and the sectors supporting people following hospitalisation.

For people with disability or complex support needs, the pathway from hospital into the community may involve the NDIS, Support Coordination, SIL, SDA, allied health, assistive technology, equipment and community-based supports.

For older people, the pathway may involve aged care, Support at Home, Transition Care, rehabilitation, dementia and complex care, residential aged care, allied health and in-home support.

Each part of the pathway operates within its own responsibilities, requirements, funding arrangements and service environment.

Effective transitions therefore benefit from the different sectors understanding each other's roles, responsibilities, requirements, limitations and practical challenges.

Bringing the Sectors together in Queensland

The Queensland Forum provides a dedicated professional environment for hospital personnel and the disability, NDIS, allied health, aged care and community sectors to communicate openly, share perspectives and better understand the challenges experienced across the pathway.

NDISDA® and Health2Ageducate® will facilitate focused professional discussions and cross-sector roundtables, providing an opportunity to identify recurring challenges, clarify points of interface and explore practical opportunities for stronger coordination.

The value of the Forum is in having the different parts of the pathway represented in the same room - hearing directly from one another and developing a stronger understanding of what each part of the pathway requires from the others.

Why your participation matters

Hospital to Home extends beyond any single organisation, profession or funding system.

A sustainable transition can depend on a number of different elements coming together - including information, funding, accommodation, equipment, allied health, provider capability, workforce, community supports and continuity following transition.

Hospital experience is an essential part of this conversation.

Equally important is the opportunity for hospital personnel to hear directly from the NDIS, disability, aged care and community sectors about the requirements and practical challenges they experience when preparing to support a person following hospitalisation.

The Forum creates the opportunity for those perspectives to come together through professional, open and constructive cross-sector discussion.
