Palliative Care Volunteers are invited to join us at the Palliative Care Volunteers Workshop on Healthy End of Planning (HELP) App
The HELP App is a new, free tool designed to activate network-centred care for people in palliative and aged care settings. Network-centred care improves end of life outcomes and the HELP App facilitates this in a practical, easy to use way.
HELP App - Network Centred Care and the Importance of Staying Connected
Why the workshop covers:
- What is network-centred care and how does it improve end of life experiences?
- How to activate network-centred care for clients and patients using the HELP App.
- Implementing the HELP Brief Intervention in your professional practice / service.
- Addressing barriers and trigger points for using the App such as digital literacy and social isolation
