Palliative Care Week Community and Health Forum

The Palliative Care Community of Northern Sydney / Northern Beaches are invited to join us in Palliative Care Week to lunch, learn, network, and codesign.

Morning Tea and Lunch Provided. 

This conference is for all members of the community, including residents along with health and community professionals. 

What the workshop covers:

• Healthy End of Life Plan (HELP) App and Network Centred Care (NCC) - Gathering your tribe to provide practical and social support at end of life  |  La Trobe University Public Health Palliative Care Unit

• Q & A with your local Palliative Care Nurse Practitioner | Northern Sydney Local Health District

• Digital Storytelling - How can we codesign, create and share stories in the community | Sydney North Health Network

• Compassionate Communities - How do we become a compassionate community in Northern Sydney and Beaches  |  Proveda

Event Highlights:

• Learn about the Healthy End of Life Planning (HELP) App, a new, free tool designed to activate network-centred care for people in palliative and aged care settings.
  
• Participate in an interactive creative self-care activity.
• Network with others in the community, and the wider palliative care team, over Lunch.
