Pathways in Community Care
Description
Supporting Mental Health in the Community; through strengthening pathways of care.
This conference will explore how we can develop and strengthen pathways of care across community mental health services. Our keynote speakers will open the day by examining the key policy drivers, national priorities and emerging learning that are shaping the future of community mental health care.
As demand for services continues to rise, community mental health teams face increasing pressures on capacity and resources. At the same time, these challenges are driving innovation, with new models of care and approaches to service delivery emerging across the sector. The conference will provide an opportunity to reflect on these developments, share learning, and consider how services can evolve to better meet the needs of local communities.
In the afternoon, we will hear directly from NHS Trusts and service leaders about their experiences of designing and delivering innovative community mental health services. Through these presentations, delegates will gain insight into practical approaches that are strengthening pathways of care, improving access, enhancing integration, and delivering better outcomes for people using mental health services.
Together, we will explore how collaboration, innovation and shared learning can support a more responsive, sustainable and person-centred community mental health system.
