LIABILITY WAIVER/HEALTH DECLARATION FORM

BEFORE THE SESSION:

Please avoid eating a heavy meal 3-4 hours before the session or any alcohol or substance use 24 hours prior.

ALL EQUIPMENT IS PROVIDED - mats, blankets, cushions, eye masks. Just bring your beautiful self.

· Wear comfortable loose clothing. No belts or jewellery.
