Event description

Advance Care Planning Community Education Session

In Person at Criterion House

108 Bathurst St, Hobart

(Located across the footpath from Mathers House approximately 20 metres. Behind the Playhouse Theatre)

Learn about 'What Matters Most' in Advance Care Planning with Palliative Care Tasmania. If you were too unwell and not able to

communicate your preferences for healthcare or personal care to others, who would speak on your behalf? And more importantly,

what medical decisions would you want them to make?

Advance care planning involves planning for your future health care. It enables you to make some decisions now about the health care

you would or would not like to receive if you were to become seriously ill and unable to communicate those preferences or wishes.

So come along and have a chat and explore 'what matters most to you'. Together we will learn about how you can start conversations with people who are important to you and how to document and record those wishes onto an Advance Care Directive.

We look forward to starting the conversation with you.
