The Stay Well Stay Home Chronic Conditions Collaborative is excited to have our first Learning Session. This is an opportunity for health services participating in the collaborative to learn how to use improvement science to test change ideas to better support people living with Asthma, COPD and/or Diabetes.
About this event: The first learning session for the Stay Well, Stay Home Chronic Conditions Collaborative
Purpose: Bring together 20 health service teams to collaborate in work to better support people living with Asthma, COPD and/or Diabetes to stay well and stay home.

29 March: 8.30am - 5.00pm
30 March: 8.30am - 5.00pm
Who should attend:
- Team members of Health Services participating in the Stay Well, Stay Home Chronic Conditions Collaborative
- Maximum of 5 participants per team to attend, determined by your Team Lead
