You can access the waiver here. Please download it, fill it out, and email it to alts@temescalmasters.org in order to complete your registration.

Also, please note we have a strict 7-day refund policy (full refund up to one week before the first session). This program has high demand with limited capacity, so we ask that those who register for the event commit to attending or let us know at least one week beforehand that they won't be able to join.
