Event description
This workshop equips Supported Independent Living (SIL) Team Leaders and House Coordinators with knowledge and skills to assist their teams to support the health needs of clients with intellectual disabilities.
Key areas covered include:
- Preventative Health: Understanding the importance of preventative health measures.
- Collaborative Relationships: Building effective partnerships with primary care providers.
- Annual Health Assessments: Supporting and facilitating Annual Health Assessments.
- Palliative and End-of-Life Care: Providing support for clients with intellectual disabilities at the end of life

Key Learning Outcomes:
- Promote preventative health care for the person and an awareness for how to advocate for reasonable adjustments for the person attending health appointments.
- Building confidence in collaborating with GP practices to facilitate effective annual health assessments.
- Play a proactive role in managing health plans and the persons health care goals
