We’re Better Together: Improving Health Outcomes for People with Intellectual Disability

This workshop equips Supported Independent Living (SIL) and Specialist Home Care Services Team Leaders and House Coordinators with essential knowledge and skills to support their staff in meeting the health needs of clients with intellectual disability.
Key areas covered include:
Preventative Health: Understanding the importance of preventative health measures.
Collaborative Relationships: Building effective partnerships with primary care providers.
Annual Health Assessments: Supporting and facilitating Annual Health Assessments.
Palliative and End-of-Life Care: Providing support for clients with intellectual disability at the end of life.
Oral Health Care: Recognising oral disease, understanding its link to general health, and supporting strategies to improve oral health.
Key Learning Outcomes:
Promote preventative health care for the person and an awareness for how to advocate for reasonable adjustments for the person attending health appointments.
Building confidence in collaborating with GP practices to facilitate effective annual health assessments.
Play a proactive role in managing health plans and the persons health care goals
